A Note From Emily:

I am conducting a research study entitled “Educational Program to Prevent Long-Term Complications for Individuals With Diabetes.” The purpose of the study is to design and tailor an educational program on diabetes self-management from an occupational therapy perspective. The following survey will be used to gather firsthand insight into the daily lived experiences of people managing diabetes, identifying their specific challenges, support needs, and gaps in current care. The data collected will guide the development of educational programs that are individualized to each patient, ensuring the program is patient-centered, practical, and impactful.

Following completion of the survey, you may also choose to participate in the educational program. There is no obligation to participate in or complete the program after the survey. You may decide the level of involvement that works best for you, and you may withdraw at any time. If you complete the program, medical data such as CGM information and Diabetes Distress Scale scores may be used for research purposes. All information will remain confidential and de-identified in the research findings.

To qualify to participate, you must be a patient at Lauren Newman Nutrition Therapy and have a diabetes or prediabetes diagnosis.

Completing the survey should take around 5 minutes of your time.
