If you want to feel less stressed around diabetes and more confident in your management, we can help!

What do we mean by “Diabetes Education”?

Diabetes Education provided by a Certified Diabetes Care and Education Specialist (CDCES) can be an incredibly powerful tool in your care. CDCES clinicians are diabetes experts, trained to support you in navigating life with diabetes.

Our team is unique: we can provide eating disorder and Health at Every Size informed diabetes specialized support to clients at all stages of their diabetes journey, with any kind of diabetes. We will help you learn about your diabetes, teach skills you need for diabetes related self care, assist in crafting a personalized and sustainable care plan, support you in coping with the challenges of diabetes, and coordinate care with your doctors/advocate on your behalf.

We highly recommend connecting with one of our CDCES providers if:

  • 60-75 minutes

Our first appointment together is a chance for us to get to know each other better! We’ll take a deep dive into your past and present diabetes care plan. We will explore what type of support you need to meet your goals, answer your questions, and establish a plan for moving forward.

2

  • 30-50 Minutes

Follow up sessions are where we’ll do the hard work together. During follow up sessions, we will reflect on progress, celebrate wins, and unpack struggles to help you achieve your goals. We will provide you with tools, education, and structure to support you through the inevitable ups and downs of this journey.

The frequency and duration of follow up sessions can vary. Most of our clients benefit from weekly check ins to start, while others meet with us less frequently. Our goal is always to work towards a place of independence, where you need us less and less! As you feel more confident and are making progress, your sessions with us will decrease in frequency and/or duration.

We will collaborate with you and any other healthcare providers on your team to ensure you are receiving collaborative and comprehensive care.

Fees and Insurance

Insurance

We do not currently accept any insurance plans. We encourage all clients to explore their out-of-network benefits within their individual policies. We can provide a superbill for you to submit to your insurance company for reimbursement, but please note that the client is responsible for full payment at the end of each session and a superbill does not guarantee reimbursement from insurance companies for sessions.

Not sure if you have out of network benefits? No worries! We have a handy script you can use to call your insurance company and get some information! Click here to download the script now!

Fees

Initial assessments are 75 minutes and follow up sessions are 50 minutes. Lauren charges $250 for an initial assessment and $200 for follow up sessions, Maria and Kelly charge $165 for initial assessments and $140 for follow up sessions. Accessibility is important to us: we offer reduced fees for clients who need it. Please contact us to let us know if you would need a reduced rate fee for sessions and we will do our best to accommodate you, or provide you with referrals to providers who can.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059.
